Reducing Delayed Discharges and Transfers of Care

Our NHS Improvement Programme, Reducing Delayed Discharges and Transfers of Care (DTOC), continues to progress. Our original aims were:

  • To reduce medically fit for discharge numbers by 50%
  • To create a sustainable long term fix
  • To have a single consistent approach
  • To create a set of Bay standards and site based delivery
  • …. And to build ownership across all partners, working together

During our four day session with partners a couple of weeks ago, we developed ten work streams. Each one continues to progress, and in addition to setting up weekly operational meetings, we aim to make a film each week to update you all on what’s been happening.

Essential telephone upgrade to take place

    I am a Councillor and at a Community meeting this morning an octogenerian said how she was worried as the winter begins to arrive because she is not looking forward to hospital appointments. Due to the lengthy waits at an uncovered bus stop which has no seating where some times one has to wait for nearly an hour in order to catch the number 6 bus back to Ulverston.

    As we are encouraging folk to stay independent I think that is an issue I should like to bring to your attention.

    I did mention the WRVS driver and the County Council Volunteer schemes….do you know of any other transport solutions that may apply for said person who is very slight and uses a stick.
